Dear Sir/Madam,

I am writing to bring to your attention a matter of great concern to me. My name is Manpreet, and I am a resident of Moga. Unfortunately, my identity card has been stolen, and I am writing to request your assistance in resolving this matter.

The incident occurred on 12-1-2023 when I realized that my wallet and identity card were missing. I immediately searched my premises and made inquiries with the people I had been in contact with, but to no avail. I believe the card may have been stolen while I was out and about in the city.

The loss of my identity card is causing me great inconvenience and distress. It is an essential document for many of my daily activities, including accessing my bank accounts, proving my identity for work, and voting in elections.

Given the sensitive information on my identity card, I am also concerned about the potential misuse of my personal details by the person who stole them.

I would be most grateful if you could advise me on what steps I should take to report the theft and obtain a new identity card. Do I need to file a police report, and what is the process? Are there any specific forms I need to fill out or any documentation I need to provide?
